Why the most common codes aren't the most expensive Copy link

Rank the same 500 codes by patients and then by dollars, and the two lists disagree. A cheap service given to millions and a costly one given to thousands can land far apart depending on which question you ask.

What this data covers—and what it doesn't Copy link

These are CY2023 Medicare fee-for-service totals at the national level. Medicare Advantage, Medicaid and commercial volume are excluded, so real-world use of every code here runs higher than the numbers show.

Beneficiary counts belong to a single code. Summing that column would count the same patient once for every code they received, so this page never totals it or calls it a patient count.

The shortlist covers numeric CPT codes. Drug codes and HCPCS Level II are excluded by design — the first are billed per unit, which makes service counts incomparable, and the second already carry CMS-owned descriptions.

8 codes on this list have since left the fee schedule. CMS billed them heavily in CY2023 and then deleted them, so their links point at the last quarter that priced them rather than at a page that no longer exists.

How the ranking is built Copy link

A code's overall position is its best standing across two measures: unique beneficiaries served and total allowed dollars. Ranking on either one alone buries half the story—beneficiaries favor cheap high-volume tests, dollars favor expensive procedures. Sort the table by a single measure to see each view on its own. The ranking spans all 7,873 CPT codes billed to Medicare in CY2023; this table shows the top 500 we cover.
