The wider coverage takes longer on the scanner and is reserved for situations where disease is genuinely expected outside the skull-base-to-mid-thigh window. Choosing between this and the shorter study is a matter of how far the imaging field is carried, not of how the scan is performed. Think of a patient with a melanoma on the calf, or with myeloma affecting bones throughout the skeleton. The scan is carried down the arms and legs rather than stopping at the thighs, so nothing outside the usual field gets missed. You'll typically see CPT 78816 on a bill or explanation of benefits (EOB) when a clinician performs or bills for this service.
