25CLABQ3V2—the July 2025 Clinical Laboratory Fee Schedule (CLFS) files
The official CMS data behind Q3 2025 on this site—in effect July 1 – September 30, 2025. The quarterly lab fee schedule files (26CLABQ3, 25CLABQ4, …) carrying what Medicare allows for every clinical laboratory test it prices nationally—finished dollar amounts rather than RVUs—plus the rows CMS flags for MAC pricing instead.
