The staff maintain a comprehensive care plan and spend the month chasing the things that fall between appointments - coordinating with specialists, reconciling medications, arranging home services, following up after hospital stays, and taking patient and caregiver calls. What separates it from ordinary chronic care management is both the amount of time and the difficulty of the decisions involved, which have to rise above routine. Think of someone living with heart failure, diabetes, and kidney disease who sees four different specialists. Over a month, a nurse at the primary care office keeps the medication list straight between them all, sorts out a home health referral, calls after a missed lab, and talks the patient's son through a change in the fluid restriction. None of that happens in an exam room, but it is what keeps the patient out of the hospital. You'll typically see CPT 99487 on a bill or explanation of benefits (EOB) when a clinician performs or bills for this service.
