Urinalysis: CPT code and Medicare rate
A urine test used to evaluate urinary symptoms, suspected infection, or as part of a wider work-up. 81001 includes examining the sample under a microscope; 81003 is the automated dipstick reading alone.

Usually billed as
81001 has no national Medicare amount in the Q3 2026 release — its status is statutory exclusion. What Medicare pays depends on the circumstances of the individual claim.
