Urinalysis: CPT code and Medicare rate
A urine test used to evaluate urinary symptoms, suspected infection, or as part of a wider work-up. 81001 includes examining the sample under a microscope; 81003 is the automated dipstick reading alone.

Usually billed as
81001 has no national Medicare amount in the Q4 2026 release. Its status is statutory exclusion, and what Medicare pays depends on the circumstances of the individual claim.
