In plain terms: A blood test for antimicrosomal antibodies, the thyroid autoantibodies more commonly referred to today as thyroid peroxidase antibodies. Their presence indicates that the immune system is attacking the thyroid gland, which is the underlying mechanism in Hashimoto thyroiditis and is also frequently seen in Graves disease. The test is reported per antibody tested, so a broader thyroid autoimmunity workup may involve more than one. Think of a patient with fatigue, weight gain, and a raised TSH. Knowing the thyroid antibodies are positive tells the doctor the gland is under immune attack rather than failing for some other reason, which sets expectations that thyroid function will likely keep declining and need lifelong replacement. You'll typically see CPT 86376 on a bill or explanation of benefits (EOB) when a clinician performs or bills for this service.
