What is a 84154 visit in patient-friendly terms?

In plain terms: Measures the portion of prostate specific antigen circulating unattached to blood proteins, which is then expressed as a percentage of the total PSA. The reason anyone cares about the split is that prostate cancer tends to produce a lower proportion of the free form, while a benign enlarged or inflamed prostate tends to produce a higher one. It is a second-stage test rather than a screen: it is run when a total PSA has come back in an ambiguous middle range and the decision at hand is whether to proceed to biopsy. Think of a man whose screening PSA came back a little high, with a normal exam and no symptoms, facing the choice between a biopsy and watchful waiting. This test refines the odds enough to make that decision less of a coin flip. You'll typically see CPT 84154 on a bill or explanation of benefits (EOB) when a clinician performs or bills for this service.
