In plain terms: Removal of an existing cardiac contractility modulation (CCM) pulse generator and implantation of a new one in the same operative session - typically a routine, planned battery-depletion swap. The existing leads are reused rather than replaced. It differs from the nearby generator-placement code, which is used when a new generator is placed without an old one being removed at that same visit. Think of a patient whose CCM device has been working well for years but whose battery is now depleted. In a single, relatively brief procedure, the old generator is removed from its chest pocket and a new one is connected to the existing leads. You'll typically see CPT 0414T on a bill or explanation of benefits (EOB) when a clinician performs or bills for this service.
