# CPT 99345

> Medicare paid $218.02 for CPT 99345 in the office (non-facility) setting and $218.02 in a facility under the Q4 2022 Physician Fee Schedule, effective October 2022. These are the national allowed amounts before the ~2% sequestration cut and before locality (GPCI) adjustment. This price has been unchanged since Q1 2022, when it decreased 1.1%.

## What it is

The most demanding first-time house call, for a patient new to the practice whose condition requires high-complexity decision-making — severe or unstable illness, several serious conditions interacting, or a problem that could quickly become life-threatening. The clinician does a full evaluation in the patient's own home and builds an initial plan. It is the longest of the new-patient home visit levels.
