## What it is

Removal of a clouded natural lens and implantation of an artificial one, in an eye that makes the operation genuinely harder than routine. The surgeon still breaks up and removes the cataract and places a lens implant, but has to add steps ordinary cases do not need — stretching a pupil that will not dilate, staining a dense white cataract so the capsule can be seen, or bracing a weak or torn capsule with a support ring or sutures. What sets this apart from standard cataract surgery is the difficulty of the eye and the extra hardware and technique required, not a different end result.
