# CPT 17004

> Medicare paid $161.68 for CPT 17004 in the office (non-facility) setting and $100.69 in a facility under the Q4 2020 Physician Fee Schedule, effective October 2020. These are the national allowed amounts before the ~2% sequestration cut and before locality (GPCI) adjustment. This price has been unchanged since Q1 2020, when it increased 4.1%.

## What it is

Treatment of a large crop of precancerous skin lesions, usually actinic keratoses, in a single session, where fifteen or more spots are frozen or otherwise destroyed. It replaces the per-lesion counting used for smaller numbers: when the count reaches this threshold, the whole session is reported with this one code instead. As with all destruction codes, the lesions are killed in place and no tissue goes to pathology.
