# CPT 17004

> Medicare paid $152.17 for CPT 17004 in the office (non-facility) setting and $102.40 in a facility under the Q1 2016 Physician Fee Schedule, effective January 2016. These are the national allowed amounts before the ~2% sequestration cut and before locality (GPCI) adjustment. This is a 0.1% increase from the previous quarter.

## What it is

Treatment of a large crop of precancerous skin lesions, usually actinic keratoses, in a single session, where fifteen or more spots are frozen or otherwise destroyed. It replaces the per-lesion counting used for smaller numbers: when the count reaches this threshold, the whole session is reported with this one code instead. As with all destruction codes, the lesions are killed in place and no tissue goes to pathology.
